ASM performance year 1 starts January 1, 2027

Connected intelligence.
For the care between visits.

IHX for ASM Specialists brings personal health intelligence, patient guidance and practice coordination into one connected experience for heart failure and low back pain. LOLA, your patients’ personal intelligent AI assistant, helps make the next step easier to understand.

WHY NOW

Specialty care extends
beyond the appointment.

CMS’s Ambulatory Specialty Model begins January 1, 2027, for selected specialists caring for people with heart failure or low back pain. It evaluates quality, cost, improvement activities and interoperability.

IHX is designed to help connect the work around the patient: understanding the plan, completing next steps, sharing relevant information and preparing the care team for what needs attention.

The questions every practice asks

Straight answers before January 1.

01

“How much of my Medicare revenue is really at risk?”

Dr. Karen Lee, general cardiologist

Up to 9% of all your Part B payments in 2029, rising to 12% by 2033. IHX shows your number and how much of it you can protect.

02

“How do I hit the quality measures without hiring more staff?”

Dr. Sam Okoro, interventional pain

LOLA collects patient surveys, IHX flags gaps before each visit, and your data stays ready to report.

03

“What exactly do we have to do with primary care?”

Maria Santos, practice administrator

A referral connection that includes social needs screening, and a written collaboration protocol. IHX helps set up both.

04

“We already have a registry. Why would we need IHX?”

Dr. Sam Okoro, interventional pain

A registry reports results. IHX works with your patients between visits to change them, and can feed your registry.

How an IHX launch works

Step 1

Free readiness check: clinicians matched to the list, your at-risk revenue, and gaps by category.

Step 2

Gap worklists and LOLA set up for patient surveys and check-ins, with your EHR connected where available.

Step 3

Primary care partners ranked, arrangements drafted, and the referral loop switched on.

Timing depends on your practice, data access and EHR. We agree a schedule with you after the readiness check.

FAQ

Good questions.

Is IHX a registry or a reporting vendor?

No. IHX works between visits to change results: patient engagement, gap closure, cost insight and primary care coordination. It can feed the registry or reporting tool you already use.

Does IHX replace our EHR?

No. IHX connects to your certified EHR and pulls records from other health systems into one view. Your EHR stays your system of record.

How fast can we be ready?

Timing depends on your systems, data access, clinical workflows and agreed pilot scope. We will define the launch plan with your team.

How do you handle patient data?

Data access, consent, business associate agreements and clinical responsibilities must be defined with your practice before deployment.

We're not on the final list. Is this still useful?

Yes, if you're a primary care practice partnering with ASM specialists, or a group preparing for value-based programs. The same tools close gaps and lower episode costs either way.

What does it cost?

Readiness assessment scope and pricing will be confirmed. Launch Program pricing depends on the number of ASM clinicians, with early adopter pricing for groups that start before launch. Book a briefing for a quote.
